I HEREBY CONSENT to the use of my. name, likeness, voice and poses and do hereby grant to WTVP, licensed to Illinois Valley Public Telecommunications Corporation – 101 State Street, Peoria, Illinois 61602 (herein known as WTVP), its agents, successors and assigns, the right, but not the obligation, to use, exploit, transmit and/or exhibit this electronic recording or any audio and/or visual elements thereof, for an unlimited number of uses throughout the world in perpetuity in all media whether now known or hereafter known or devised and in all advertising, promotion, and publicity associated therewith; in connection with ANY WTVP production.

I ACKNOWLEDGE that WTVP owns all right, title, right and interest in and to this electronic recording, including, without limitation, the worldwide copyright therein.

AND I DO HEREBY RELEASE and hold harmless WTVP, its agents, directors, officers, employees, successors and assigns from and against all manner of action, causes of action, liabilities, claims and demands whatsoever which I may have or which my child may have or which my or my child’s heirs, executors, personal representatives or assigns or any of them shall have arising out of the use of this electronic recording by WTVP, its agents, successors, licensees and assigns.

WTVP has the right to edit, cut, add or delete from the original scenes. I hereby waive on behalf of my child any moral right which she/he may have in this electronic recording or any WTVP production.

I HEREBY WARRANT that I am a parent / guardian of this child and have every right to contract in his/her name in the above regards. I state further that I have read the above consent and waiver and that I am fully familiar with the contents herein. I acknowledge that I have executed this Consent and Waiver freely and voluntarily with full understanding of its contents. I have not relied on any representations made by WTVP except as specifically set forth herein.

THE TERMS OF THIS CONSENT AND WAIVER shall be binding upon me, my child and my child’s heirs, executors, personal representatives, successors and assigns.
